Premise: A new action-thriller game with trained soldiers and an arsenal of advanced, powerful, modern day firepower whom try and take down enemy combatants. The brutal opposition are threatening the world in various settings and scenarios. A story with twists and turns about soldiers fighting; using both ground-war technology and aerial strikes on a battlefield where speed, accuracy, and communication are the major factors in victory. Genre: Call for Action is a first-person shooter (FPS) which the person’s cross hairs are placed in the center of the screen and everything happens around it. Platform: Call for action has four different platforms. It is compatible with Microsoft Windows, PlayStation 3, Xbox 360, and Wii. The best platform for this game is PlayStation 3, because of its superior graphics software, easier controlling, and less hardware malfunctions. These reasons have been approved and guaranteed by users. The second best platform for this game is Xbox 360. It can handle the graphic and it’s easy to control, like PlayStation 3. Microsoft Windows and Wii are the other platforms which can be used in order to play Call for Action. Backstory: The story takes place in the year 2011, where several U.S. Sergeants and British SAS members find a nuclear weapon on a ship traveling to Africa. After the evacuations, they find evidence of ties between Germans and Russians. At the same time, the Russians want to distract people in order to achieve their goal. So they hire a local separatist leader, called Ivan, to execute the president of the United States to keep the military away. USMC 1st Force Recon team must find and rescue the president, but during the operation, United States Central Command is notified by Seal Team Six of a German nuclear weapon. Later on British SAS find and interrogate Ivan, he exposes that the German leader supplied the nuclear bomb in order to start World War III. Therefore, A combined operation, by U.S. Sergeants and British SAS members, is taken on to stop German leader. Target Rating Call for Action’s rating is M which stands for Mature. Content is generally suitable for ages seventeen and up because the content contains intense blood and gore, intense violence, and strong language. Target Market This game is originally for residents of the US, but later it can be available to other countries. The target audience is mostly teens generally above the age of 17. On the other hand, the game is only rated M because of parents not wanting their children playing a game with some blood in it. A game should only be rated M if it has sexual scenes not because of blood. Player Motivation This game has two versions, the fist version is, playing the normal mode which the player goes through the story and tries to finish the game, and the second version is the online version which players will compete with their friends in an online mode. Call for Action has a lot of motivations. First, the game finishing scene is the player needing to find the Germans leader in Africa and kill him. If the player kills him, he/she has the right to destroy the nuclear weapon. Second, in this game, not only you can kill people, but you can also drive a tank or a car. Third, when the player finishes the story, the game will give the player a set of special weapons for the online gaming. UPS There are some unique things in the game and about the game which makes it special. First, in this game players can shoot with their weapons, drive a car, or play as a pilot. (Drive an airplane) Second, it has a very high quality and graphic which gives the real feeling of the game. Third, it uses the PlayStation’s dual shocks, so every time players get hit, it will start shaking. The last reason that makes this game special is that players can check their friends’ progress and follow them. Therefore, this game helps the players to experience the war situations and teach the necessary information and acts that people need to do during the war. Competitive Analysis There are three other games which could be considered competitors of Call of Action. The first game called Battlefield 3. This game is a first person shooter by a Swedish developer. According to Wikipedia, â€Å"In-game, the European Union and the United States fight China and the Middle Eastern Coalition. It is known that in the game’s story, the EU and the US are allies and the EU has negotiated a peace deal with Russia.† The platforms for this game are same as Call for Action. (Microsoft Windows, PlayStation 3, and Xbox 360) The second game is Call of Duty Black Ops II which was developed by Treyarch. According to Wikipedia, â€Å"Black Ops II is the first game in the Call of Duty franchise to feature future warfare technology and the first to present branching storylines driven by player choice. It also offers a 3D display option. In this game the player campaign features two connected storylines, with the first set from 1986 to 1989 during the final years of the first Cold War, and the other set in 2025 during a second Cold War.† The platforms for this game are PlayStation 3, Xbox 360, Microsoft Windows, and Wii. The third game is Blacklight which was developed by Zombie Studios. According to Wikipedia, â€Å"Zombie is planning to make Blacklight a multimedia franchise that will include the video game, a feature film, and a trilogy of comic books. The idea was concepted by Zombie executives which later pitched the idea to several film and comic book production companies.† The platforms for this game are Games for Windows – Live, PlayStation Network, and Xbox Live Arcade. Based on the other three descriptions and Call for Action description, Call for Action has more options and futures. It may not be better than Call of Duty Black Ops II, but has the same quality and features. In this game, you can fly a plan, drive a car, or a tank. It will use the dual shock in order to give the real feelng to the player. It will attract players to finish the story by giving them prizes at the end. Therefore, it can be one the top competitions in the market. Goals The goal of this game is to give a player the time to enjoy, learn, and challenge themselves. In this game people can spend time and put themselves in a war situation. What needs to be done? How to control a gun? How to communicate with people in an emergency situation? Players can learn the name of all guns and get to know their specifications, how they are working and what they are good for. Also in an online game, people can compete with each other. Players can see their friends’ progress and play with them, or players can invite their friends, gather a team, and fight with other people. In this case, people will communicate with each other and learn a lot about the game by asking or watching other people playing.

From a theoretical point of view, this research paper introduces a literature review on the application of back propagation algorithm of an artificial neural network, linear probability model, and binary choice (logit probit) model for credit risk management. Whereas, from an empirical point of view, this research compares the econometric models and artificial neural network using Mongolian banks’ credit risk data, and shows the differences between the aforementioned four models. We demonstrate that artificial neural network model is more convenient for Mongolian banks’ credit risk management than other econometric models due to the models’ evaluation and forecast accuracy. Therefore, we recommend Mongolian banks and financial institutions to apply ANN model to forecast credit risk and to hedge risk. Not breast feeding, hormones inherited from parents, hormones that mother and child may have been exposed to through foods that were ingested during pregnancy, or exposure to toxins in commonly used household items like plast ic spoons and plates. Further, some items used to feed babies like their bottles may contain some small traces of toxins, which are ingested during feedings. Retha Newbold (Bagley, 2009), of The National Institute of Environmental Health Sciences (NIEHS) in North Carolina believed that emerging evidence shows being overweight is not just the result of making poor food choices, or lack of activity, but that exposure to environmental chemicals during development maybe contributing to the obesity epidemic in America. There is no doubt that while a woman is pregnant exposure to harmful chemicals can cause serious illnesses that become more apparent years and even decades later. It is not far-fetched to believe that fetuses and infants can be predisposed to obesity through hormones and chemicals. In a study conducted by Newbold, lab mice were injected with normal or the same levels of estrogen as people are generally exposed to in the environment. The study showed that in six months the mice that were exposed to the estrogen was 20 percent heavier and had 36 percent more body fats than mice that were not exposed to the estrogen. Newbold was made sure that both the exposed and the controlled mice in the study were fed the same amounts of food and were exposed to the same amount of activity. These implications are huge, and this study shows that there is a direct correlation between some hormones and obesity in infants. Bruce Blumberg (Bagley, 2009), a scientist at the University of California, Irvine said "there is a correlation between obesity and a lot of things, and that finding a link between hormones/ chemicals didn't mean much (Born To Be Big, 2009)." Structural Functionalism and Conflict Theory Essay Karl Marx and Max Weber were the first conflict theorists in the 19th and early 20th centuries. Following Marx and Weber were three mid-20th century conflict theorists: Lewis Coser, Ralf Dahrendorf, and Randall Collins. Coser draws his theoretical ideas from Simmel. Like Simmel, Coser maintains that conflict is healthy for society. In contrast, Dahrendorf combines theoretical ideas from Marx and Weber. Dahrendorf sees power as the main feature in all social relationships. However, Collins incorporates Weber, Durkheim, and Goffman’s theories to provide a micro-level orientation to conflict theory. Collins also used geopolitics at a global level to examine political conflicts historically and geographically. According to Marx and Weber, the root of most social conflict comes from an unequal distribution of class, status, and power, as well as a group’s sense of deprivation caused by class (Allan, 2007). Coser, Dahrendorf, and Collins added to Marx and Weber’s theories. These conflict theorists assert that the degree of deprivation is essential in creating class consciousness and critical awareness. In particular, Coser discusses the consequences of inter and intra group conflict. Internal conflict can build up over time between groups and become explosive. Internal groups have a psychological need to be in conflict with each other. Modes of releasing hostility and developing authority with a corresponding justice system are necessary for healthy internal conflict. Further, external conflicts between groups create well defined and guarded boundaries to distinguish membership. Group membership becomes exclusive, which is necessary for group survival. â€Å"Conflict sets boundaries between groups within a social system by strengthening group consciousness and awareness of separateness, thus establishing the identity of groups within the system† (Coser, as quoted in Allan, p. 219, 2007). Coser maintains that conflict can have functional consequences. Some functional consequences of conflict are: social change, innovation, and increased centralized power. Structural Functionalism was the dominant theoretical approach in the United States from the 1930s through the 1970s. Structural Functionalism asserts that the various parts of society are interrelated and form a complete  system. â€Å"Just as the body is a system with specific parts (e.g., arms, legs, liver) that ensure its overall functioning, so, too, society is a system with specific parts (family, government, economy, religion, etc.) necessary for its very survival† (Appelrouth and Edles, 2008, p. 349). Two key structural functionalism theorists are Talcott Parsons and Robert Merton. Parsons theories were highly abstract. Parsons developed a social action theory to explain why people behave the way they do. He explained human actions as a result of three systems: social systems, personality systems, and cultural, and behavioral systems (Appelrouth and Edles, 2008). These systems are not separate entities; rather together they form a simplified model of society. â€Å"Social systems, personality systems, and cultural systems undergird all action and all social life† (Appelrouth and Edles, 2008, p. 352). Parsons applied his theory to the American family in Sex Roles in the American Kinship System (1943). Parsons proclaimed that â€Å"many women succumb to their dependency cravings through such channels as neurotic illness or compulsive domesticity and thereby abdicate both their responsibilities and their opportunities for genuine independence† (Parsons, 1943 as quoted by Appelrouth and Edles, 2008, p. 382). Sex Roles in the American Kinship System (1943) incited criticisms as Parson endorsed traditional gender roles, and asserted that dire consequences would occur if these roles were breached. Robert Merton’s theoretical influences were broad. He read extensively, and there are elements of Durkheim, Weber, Simmel, and Marx in his theories. In contrast to Parson’s abstract theories, Merton was a middle-range theorist. Middle range theories â€Å"lie between the minor but necessary working hypotheses that evolve in abundance during day-to-day research and the all-inclusive systematic efforts to develop a unified theory that will explain all the observed uniformities of social behavior, social organization, and social change† (Merton, as quoted by Appelrouth and Edles, p. 383). Merton was best known for his distinction between manifest and latent functions. Whereas manifest functions are the conscious intentions of the actor, latent functions are the unintended consequences of the action. Merton used the example of the Hopi rain dance to illustrate manifest and latent functions. The manifest function of the rain dance often does not  produce rain, and is called superstitious by some. However, the rain dance continues to be performed for a function that the actor is unaware of, which is the latent function (Appelrouth and Edles, 2008). â€Å"†¦This behavior may perform a function for the group, although this function may be quite remote from the avowed purpose of the behavior† (Merton, 1949 as quoted by Appelrouth and Edles, 2008, p. 391). Even though the Hopi rain ceremony does not produce the manifest function of producing rain, the ceremony does bear latent functions that sociologists and anthropologists can study. Parsons Sex Roles in the American Kinship System (1943) received a lot of criticism in the 20th century. This 20th century criticism stems from Parsons endorsement of traditional gender roles, and the dire consequences that would occur should these roles be breached. Interestingly, the 21st century critiqued the 20th century critiques. The 21st century does not claim that Parsons assertions were not sexist, rather that the 20th century critiques were sexist (Appelrouth and Edles, 2008). As Appelrouth and Edles (2008) note, Parson’s pivotal premise was not sexist at all. Parsons believed that changes that are functional for one part of the system will produce changes that are not necessarily functional for other parts of the system. Appelrouth and Edles (2008) further noted the sexism in the 20th century in the premise that women could enter the workforce without significant changes being made to other social structures and systems, and without a major increase in quality dayca re and childcare facilities. How would Parsons view 21st century families? I maintain that Parsons might find 21st century families dysfunctional. Twenty-first century families do not have the rigid structure that Parsons describes. Parsons wrote from a 1940s, 1950s white male middle class perspective, and could not picture the perspective of other social classes. However, families today are socially and racially diverse, especially in the US, and even the white middle class family does not fit Parsons proscribed role definitions. In the 21st century, there is substantial role confusion in families. Gay and lesbian couples adopt children and raise them with same sex parents. More fathers stay at home to take care of their children while the mother is the primary breadwinner. Increased educational opportunities for women have produced a professional class of women who sometimes earn substantially more than their  husbands. Families in the 21st century are also deciding to have fewer children, and are marrying at a later age (and sometimes not at all). In short, the 21st century has produced more: women in the workforce, single parent households, stay-at-home dads, same sex parents, and racially blended families. It is arguable whether families in the 21st century are dysfunctional. Certainly there are dysfunctional families; yet, many families perform well in the new social order. Would Parsons view the 21st century change in sex roles as negatively affecting the family? Indeed, some would argue that changed sex role definitions create confusion, and affect the family and the socialization of children negatively. However, sex role changes in the 21st century are logical when one looks at the situation using Parson’s AGIL scheme. The 21st century has produced rapid changes, and as a result, family roles have had to change in order to adapt to the new environment. When querying a social organization as to whether they achieved their program goals or intended purpose, the answer received in the manifest function gives little depth of information. Granted, this type of manifest inquiry is important, as it reveals whether programs have achieved their stated goals. Nevertheless, this manifest inquiry can be achieved simply by questioning agency personnel. On the other hand, utilizing latent inquiry, the assessor can discover deeper theoretical problems. By simply examining manifest functions, the evaluator becomes merely a recorder and transcriber of  behavior and actions. In contrast, latent inquiry provides more depth to an assessment. Latent inquiry discovers unrecognized information that cannot be obtained by a manifest inquiry. Latent inquiry discovers consequences, motivations, and paradoxes, which provide useful assessment information. Our research is based on the hypothesis that why people in Pakistan prefer KFC over Macdonald and we found that due to good quality and taste of food people prefer KFC . When KFC was introduced in Pakistan the prices was not affordable by people but now they have lower their prices. One reason was also the impressive packages due to which people prefer KFC over McDonald’s. While conducting our research we also came to know that KFC is more advertised this is one of the reasons that people prefer KFC over McDonald’s. Hence in general we conducted this research because we wanted to know that which brand is preferred and what is the reason behind it. 1. 4 SAMPLING In sampling plan we draw sample, sample is basically a segment of the population selected for research to represent the population as a whole. In our research we targeted people from 18 to 30 . So we approached people who visit KFC and McDonald’s . Our sample size is 40 from the whole population . Our research instrument for collecting primary data is Questionnaire in which we ask twenty questions, all are close ended for accessing people’s preference. 1. 5 METHODOLOGY The empirical analysis of the â€Å"International fast food chains in Pakistan† was based on the primary data collected through personal observations by visiting KFC and McDonald’s located in the twin cities of Rawalpindi and Islamabad. The data were collected by filling questionnaire during rush hours in the afternoon between 12:00 noon to 2:00 p. m. In all, 40 customers were accounted for during the survey time period in KFC and McDonald’s. The data were collected during the first week of January 2010. 1. 6 LITERATURE REVIEW First we will take into account the history of KFC. The company was founded as Kentucky Fried Chicken by Colonel Harland Sanders in 1952, though the idea of KFCs fried chicken actually goes back to 1930. The company adopted the abbreviated form of its name in 1991. Perfecting its secret recipe of 11 herbs and spices in 1939, KFC has come a long way. With over 10,000 outlets in the world, KFC has maintained its title, for the last 60 years, of being the chicken Experts. Now we will discuss the history of McDonald . At one time it was the largest global restaurant chain, but it has since been surpassed by multi-brand operator, in the competitor KFC is on top of the list. The business began in 1940, with a restaurant opened by brothers Dick and Mac McDonald in San Bernardino, California. The present corporation dates its founding to the opening of a franchised restaurant by Ray Kroc, in Des Plaines, Illinois, on April 15, 1955. Now McDonalds is present in 120 countries with about 32000 outlets. Market entry in Pakistan was not a big problem for KFC as it is a well-known international brand. In that stage they did promoted through their own brand. Their promotion statement was â€Å"KFC in Pakistan†. Opening the first KFC outlet in Gulshan-e- Iqbal in 1997. KFC wore the title of being the market leader in its industry. Serving delicious and hygienic food in a relaxing environment made KFC everyone’s favorite. Since then, KFC has been constantly introducing new products and opening new restaurants for its customers. Presently KFC is branched out in nine major cities of Pakistan (Karachi, Lahore, Gujranwala, Sukkur Muree) with more than 45 outlets nation-wide. Apart from fulfilling our commitment of serving delicious, fresh and hygienic food and at the same time providing our customer with the ultimate entertainment; KFC also plays in the economics development of our country. Presently KFC has provided to over 1200 Pakistanis, which adds up to 6000 individuals directly dependent in KFC Pakistan. In introduction stage KFC entered the market using market-skimming strategy. Their products were high price and targeted only upper class. Later they introduced packages which were affordable by many people. McDonalds Pakistan opened its doors in September 1998 at Lahore and presently operating in seven major cities with a network of 20 restaurants. With a strong belief in the Ray Krock phrase when you are green you are growing, McDonalds Pakistan has an aggressive plan to expand in all other cities of Pakistan and is rapidly growing with the focus to provide friendly and quick service restaurant experience to their customers. Today millions of Pakistanis place their trust in McDonalds everyday to provide them with food of a very high standard as well as good service. In the past five years, the response to McDonald’s coming to Pakistan has been overwhelming. McDonald’s located in Pakistan are currently employing about 1,000 Pakistanis and each store is managed by Pakistani managers. They have also contributed in other ways than food service. McDonalds has investment of over Rs 300 crore in the country and payment of taxes and duties amounting to more than Rs 100 crore. Since McDonald’s is doing so well in Pakistan they will continue to grow and add more stores in more cities in the future. But in Pakistan McDonald’s is facing a tough competition by KFC. KFC and McDonald’s serve to dominate the fast food business in Pakistan. Both fast food chains serve all their customers effectively, given the different categories and factors that determine the success of restaurants and fast food chains in Pakistan. However, based on the findings of the previous research, it can be perceived that KFC is running a more successful business in Pakistan, compared to the business run by McDonald’s. This was given proof in the article, based on the many and varied responses of customers to surveys, which involve their evaluation of both fast food restaurants, based on a number of factors, namely, food, service, environment, price, convenience, brand, and promotion. In terms of food, although both fast food restaurants almost have the same varieties, certain differences can still be recognized. Both KFC and McDonald’s offer chicken and hamburger meals including snacks, desserts, and drinks. However, only KFC offers a wide range of healthy side dishes, such as mashed potato, carrot muffin, and lotus fresh vegetable side dishes. In this sense, KFC aims to uphold healthy eating and nutrition of their customers. In addition, it has been emphasized in the case study that KFC appeared to have the upper hand over McDonald’s in terms of introducing new products in the market. Most of the new products of KFC, such as its lunch items, suit the tastes of the Pakistani. KFC has also been introducing new products in the market in a regular rate. In terms of environment and service, although both fast food restaurants appeal to their market due to their convenient lighting, cleanliness, music, and decorations, still, KFC serves to have the upper hand over McDonald’s. Based on the survey, more customers are â€Å"Very Satisfied† with the services and environment of KFC over McDonald’s. This is because the waiting time in McDonald’s is too long, compared to KFC. In addition, it can also be perceived that KFC has more advantage than McDonald’s, as the employees in KFC have less time to interact with customers than its counterpart, as the volume of its business is larger. The prices of food offered by both restaurants equally serve each other. However, the food offered by KFC is slightly higher, being able to offer some meals that are not offered by McDonald’s, such as the Family Bucket, Garden Chicken Burger Meal, and Hot Spicy Wing Meal. Nevertheless, the volume of customers in all given times of day, in both weekdays and weekends is still higher in KFC than in McDonald’s. Given such figures, it can be perceived that more customers prefer KFC to McDonald’s, thus, indicative of the fact that KFC runs a more successful business than its counterpart does. Moreover, the findings of the study indicate â€Å"More low income and high income customers prefer KFC to McDonald’s, while average income customers prefer McDonald’s more than KFC† In this sense, it can be observed that KFC has a wider range of customers compared to McDonald’s, thus, indicating that more customers patronizes KFC. Furthermore, for the finale, the proof that KFC is running a more successful business than McDonald’s is indicated that not just in Pakistan KFC has an upper hand but in all over the world. 1. 7 DELIMITATIONS. The problem which we faced while conducting our research are firstly the people were not very co operative in order to fill our questionnaire and we have to convince them. The survey shows a slight difference between the advertisements of both restaurants. As they are leading fast food restaurants in Pakistan so they advertised almost equally to make their roots strong in promoting their service. According to the data 57% people think KFC is more advertised and 43% found McDonald’s is more advertised. People have positive approach towards KFC because KFC food has less chronic health risks because its meal involves appropriate amount of nutrients. Although in other aspects KFC has surpass McDonald’s but when it comes to the outlook of the restaurant 63% people like the outlook of McDonald’s and only 37% people rated KFC. After analysis it was found that 60% people’s pleasant memory is associated with KFC. 32% people have KFC. 8% do not have as much pleasant memories as others. Q14) Which brand do you think earns pleasant money? 55% people views show that KFC earns pleasant money. And in a close second place 45%rated McDonald’s. Q15) On Average how much would you expect to pay for a fast food meal? 55% people expect to pay an amount of at least Rs. 500 on fast food meal. 30% agreed on paying Rs. 1000 while only 15% want to spend Rs. 1500. The popularity of any restaurant is just not dependent upon the taste and price of food but it also depend upon the enjoyable environment which include play area for kids, happy meal and gifts. 52% people consider McDonald’s is more enjoyable but 48% people like the environment of KFC. Q17) Which fast food restaurant is more accessible? The result of this question shows an even split among both options. The increasing trend of fast food leads both restaurant branches to multiply in order to make them easy to get to. 53% people think KFC is more accessible while 47% think that McDonald’s is more accessible. Q18) Which brand offer deals which have attractive prices? The higher percentage of people found the prices of KFC more attractive than McDonald’s. According to the data 55% people found prices of KFC affordable and attractive. While that of 45% found McDonald’s prices smart. Q19) If you owned your own fast food restaurant, then which fast food restaurant Attribute you want to follow-up? The majority results show that KFC has won the race in our fast food survey. It is found that KFC fulfill the needs of its customers to some extent, so considering KFC as their ideal people would like to follow the success tracks of KFC in order to have bright future of their restaurant ahead. 75% people like to follow the attributes of KFC for their own restaurant and only 25% consider McDonald’s as their ideal. Q20) Do you think the future of International fast food chains in Pakistan is bright? Based on people answers, 85%agreed with the statement that the future of international fast-food chains in Pakistan is bright, 5% were not agreed on bright future and 10% people have no idea about this. With todays hectic lifestyles, time-saving products are increasingly in demand; one of the most obvious examples is fast food. People want quick and convenient meals as a result, consumers rely on fast food . Knowing this, fast food providers are coming up with new ways to market their products that save time for consumers. All that make sense though because that is what fast food is supposed to meancheap, fast, and tasty. 1. 10 CONCLUSION The conclusion we have drawn after conducting our research is that the problem statement which we consider in our research is confirmed . Our hypothesis that KFC is in spotlight and McDonald’s is a side show was right and people prefer KFC over McDonald’s because of their good quality food and their reasonable price, hence KFC capture a large market than McDonald’s in Pakistan. 1. 11 RECOMMENDATION AND SUGGESTION After conducting our research we come to a point that about 80% people prefer KFC over McDonald’s here are some suggestions and recommendations to both the brands. Based on our research we suggest that McDonald’s should improve the quality of their food because majority of people found McDonald’s a less healthy option. It was also found that Macdonald’s satisfies the need of people to lesser extent, so McDonald’s should try hard to make up to the expectations of people. It was found by our research that although people prefer KFC over McDonald’s but they found the packing of McDonald’s more attractive than that of KFC, so KFC should work on their packing and make it more attractive. When it comes to outlook of the restaurant, majority of people like the outlook of KFC, so McDonald’s should improve their outlook. It was also found that KFC is better advertised; hence McDonald’s should work to promote their brand in order to capture a large volume of customer. Audible Clicking Sound in Psychogenic Palatal Tremor Abstract Palatal tremor, commonly called palatal myoclonus, is a rare movement disorder characterized by rhythmic contractions of the soft palate and associated with variable objective tinnitus. It may be categorized as symptomatic, essential or psychogenic. Mostly it is symptomatic because of secondary damage in brainstem or cerebellum; rarely it can be essential in the absence of brain lesions. We describe a case of young boy, who presented with palatal tremor with objective tinnitus. Most of the movement disorders may raise suspicion of psychogenic nature and vice-versa, however, this case highlights the need of careful history and keen observation of movements. Key Words: Palatal tremor; Palatal myoclonus; Psychogenic disorder; Objective tinnitus Introduction Palatal tremor is also known as palatal myoclonus. The term palatal tremor is appropriate because it correlates better with the neurophysiological study. It is a rare movement disorder characterized by rhythmic contractions of soft palate and associated with variable audible clicking sound. It may be classified into essential, symptomatic or psychogenic type. Mostly palatal tremor is symptomatic because of secondary damage in brainstem or cerebellum. Rarely, it is categorized as essential in which no brain abnormality is identified. There have also been reports of psychogenic palatal tremor in association with other neuropsychiatric illnesses [1]. Case Presentation An 18-year-old boy born out of non-consanguineous marriage with normal birth and developmental milestone, presented with loud audible clicking sound and abnormal palatal movement for six months. These movements used to worsen during stress and improve with sleep. He had no abnormal behaviour, mood disorders, seizures or abnormal limb movement. There was no history of similar illness or psychiatric disorder in family members. The oropharyngeal examination showed abnormal high frequency (approximately 100 Hz) palatal movement on both sides [Video]. There was associated rhythmic contraction of soft palate, tensor veli palatini and levator veli palatini muscles. These movements were associated with loud audible clicking sound (tinnitus). During examination, suggestion (inducing by thinking of it) and distraction (mind diversion by concentrating on motor and cognitive functions like performing complex movements, reading, thinking or calculations) methods were used to demonstrate voluntary control of these movements. The patient was asked to voluntarily suppress and induce these movements. Surprisingly, he was able to suppress and induce these movements. Cumulative examination findings (suggestibility, distractibility, worsening during stress, suppression during sleep) demonstrated voluntary control of palatal movement. Rest of the findings including general physical, otolaryngological and neurological examinations were unremarkable. Complete blood count, serum biochemistry including transaminases, lactate and pyruvate, ammonia, ceruloplasmin and copper levels, plasma amino acids, thyroid function tests and antinuclear antibody (ANA) were normal. Slit lamp examination did not show K-F (Kayser–Fleischer) ring. Audiometry and electroencephalography (EEG), magnetic resonance imaging (MRI) of brain with thin cuts focusing on brainstem [Figure 1] and MR angiography were normal. The patient was uncooperative for electromyography (EMG) study. Psychiatric opinion including detailed psychological analysis was obtained. Finally, a diagnosis of psychogenic palatal myoclonus was made. He was counselled along with placebo therapy. He was asymptomatic at discharge. After six months followup, he is symptoms free. Discussion Palatal tremor, also known as palatal myoclonus, is an abnormal movement of the soft palate. The term â€Å"palatal tremor† is more appropriate because it corresponds better with the electromyographic (EMG) characteristics of the rhythmic movements. It may be classified into essential, symptomatic (secondary) or psychogenic. Deuschl et al described that the movement of the soft palate is due to contraction of tensor veli palatini muscle in essential palatal tremor and contraction of levator veli palatini muscle in secondary palatal tremor [2, 3]. The contraction of these muscles result in closure of eustachian tubes and produce audible clicking sound. Essential type of palatal tremor mostly occur in children. The patients with essential palatal tremor usually have an audible ear clicking sound (objective tinnitus). Although essential palatal tremor has a benign course and usually disappears spontaneously, it is very annoying for the patient as well as to the bystander due to continuous audible clicking sound. The brain is normal in essential palatal tremor. It is usually bilateral and disappears during sleep. The etiopathology of essential palatal tremor is not known. Fernandez-Alvarez classified essential palatal tremor under transient primary movement disorders of childhood [4]. The symptomatic or secondary palatal tremor is mostly seen in adult males. It is usually associated with hypertrophy of the inferior olives; however, its precise role in causing palatal tremor has not been established [5]. It can be a consequence of trauma, infections, encephalitis [6], degenerative lesions, vascular, Krabbe’s disease [7] or tumors of the cerebellum [8] or brainstem. Usually, there is no associated audible tinnitus and movements persist during sleep. It is usually unilateral and associated with neurological deficits. Many movement disorders raise suspicion of psychogenic nature and vice-versa; palatal tremor can also be a part of psychogenic spectrum. Few cases of psychogenic palatal tremor have been described in the literature [9, 10]. The closest differential diagnosis is essential palatal tremor, which may have audible clicking sound, disappear during sleep and have normal neuroimaging studies. Voluntary inhibition of palatal tremor is also possible in some cases of essential palatal tremor [11]. Psychogenic palatal tremor often have various psychiatric symptoms (anxiety disorders). These movements are characterized by variable frequency, increased during stress and attention, suppression when distracted; disappear during sleep, marked improvement with placebo and psychotherapy. During examination, suggestion (inducing by thinking of it) and distraction (mind diversion by concentrating on cognitive and motor functions like thinking, reading, calculations or performing complex movements) method s can be used on patients to demonstrate voluntary control of these movements. Our patient had acquired special motor skills to both induce and suppress voluntarily rather than inhibition of involuntary movements. These showed complete voluntary control of movements and suggested a psychogenic etiology. The underlying psychiatric illness is a conversion disorder in most of the cases described in literature. The possibility of essential palatal tremor is ruled out by detailed clinical, psychological and laboratory examination. Our patient improved abruptly on suggestion, placebo and psychotherapy. Psychogenic palatal tremor is a treatable disorder. A detailed psychoanalysis should be an essential part of management [12]. The patient should be managed with a combination of psychotherapy, anxiolytics and antipsychotic drugs. The psychogenic palatal tremor usually responds well to placebo and psychotherapy. Conclusion Palatal tremor is attributed to organic lesion of the brain; however, occasionally it may be due to psychogenic etiology. In our patient, movements were intermittent; used to worsen during attention, suppress during distraction and voluntary control on suggestion. All these features suggest psychogenic palatal tremor. Here we emphasize the detailed clinical and psychogenic evaluation of the patient and need for psychiatric treatment in these cases. Figure and Video Legends Figure 1.
